5L Performance Group ("5L," "we," "us") is an organizational performance and leadership consultancy based in Omaha, Nebraska. Information you give us
When you take the L-evate Snapshot, book a strategy call, RSVP to a 5L Huddle, or contact us, we collect what you enter: your name, work email, company, phone number, role, industry, company size, revenue range, department, your assessment answers, and anything you write about your organization's challenges.
When your organization participates in a Full L-evate Assessment, your survey answers are stored separately from your name. Your name and email are recorded only to track who has completed the assessment and are never linked to your individual responses. Results are reported to your organization's leadership in aggregate only, and groups with fewer than three respondents are not shown.
Information collected automatically
Like most websites, we collect basic usage information: pages visited, the site or campaign that referred you, and general device and browser type. See our Cookie Policy for details on cookies and similar storage.

- To run Full Assessment campaigns your organization has engaged us for

Who we share it with
We do not sell your information, and we do not share it with advertisers. We use a small number of service providers to run the site: website and database hosting, email delivery, and scheduling (Calendly, when you book a call). These providers process information only to provide their service to us. We may also disclose information if required by law.
Retention and security
We keep assessment and contact information for as long as it is useful for the purposes above, and we protect it with access controls so only 5L's principals can view it. No method of transmission or storage is perfectly secure, but we take reasonable measures appropriate to the information we hold.
Your choices and rights
You can ask us to show you, correct, or delete the information we hold about you by emailing info@5Lperformancegroup.com. You can unsubscribe from marketing email at any time. Transactional email, such as your own assessment results, is sent only because you requested it.
Children
This site is for business leaders and is not directed to children under 13. We do not knowingly collect information from children.
Changes
If we change this policy, we will update it here with a new effective date.
